The health requirements below must be submitted by October 15. As part of returning to school, families are required to provide documentation of certain state-mandated health exams and immunizations. The requirements for specific grades are listed below:

Physical Exam completed within the last year: Kindergarten and 6th Grade

Up-to-Date Immunization Record: Kindergarten and 6th Grade

Eye Exam completed within the last year: Kindergarten

Dental Exam completed within the last 18 months: Kindergarten, 2nd Grade, and 6th Grade
